The contact form emails your name, contact details, reply preference, and message to the practice through Google Workspace. Please do not include diagnoses, treatment history, or other private health information. This form is for initial inquiries, not secure clinical messaging.
Secure client services
The practice plans to use SimplePractice for secure intake and client workflows. When connected, a clearly labeled link will take you to that service, where its own privacy terms and the practice’s clinical privacy notices will apply.
Website technology
Fonts and images are served with the website. This version does not load Google Analytics, advertising trackers, or embedded maps. Clicking the directions link takes you to Google Maps, which has its own privacy terms.
Hosting and future changes
The live hosting provider may process standard request information, such as IP addresses and browser details, to deliver and protect the website. The form uses self-hosted ALTCHA verification and short-lived abuse-prevention records. Those records contain verification data and hashed network addresses, not your message. Inquiry messages are sent to the practice’s email account; the form handler does not save them in a website database. This notice will be updated if analytics or other services are added.
